Dementia Support Coach
As a Dementia Support Coach, you will be part of a dedicated team that makes a meaningful impact every day by supporting individuals living with dementia as they transition from hospital to home or into residential care.
Key Responsibilities:
* Provide on-ground support and implement relationship-based strategies for clients, carers and care staff to ensure a seamless transition.
* Model and coach best-practice approaches to help carers and aged care staff understand and support people living with dementia, promoting quality care.
* Work closely with the Team Leader, Dementia Consultant and Senior Consultant to implement recommendation plans and provide guidance to stakeholders.
* Collaborate with peers to build capability across aged care teams, ensuring person-centred care is delivered consistently.
