Job Overview
This role focuses on bridging the gap between Aboriginal individuals and families navigating mental health services. The Liaison Officer will work across hospital and community settings to facilitate culturally informed care, foster trust and understanding throughout the mental health journey.
Key Responsibilities
* Support Planning and Coordination: Assist in planning and coordination of admissions, treatment pathways, and discharge planning.
* Referral and Follow-up Coordination: Coordinate referrals and follow-up care in collaboration with multidisciplinary teams.
* Culturally Safe Care: Advocate for culturally safe and responsive mental health care for Aboriginal consumers and families.
* Service Access and Support: Engage with consumers and families to facilitate access to appropriate services and supports.
* Liaison and Support: Work across hospital and community settings to provide ongoing liaison and support.
Requirements and Qualifications
* Aboriginal Descent: Be of Aboriginal or Torres Strait Islander descent.
* Health Qualification: Hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a relevant health qualification.
* Social and Emotional Wellbeing Knowledge: Demonstrated knowledge of the Aboriginal concept of social and emotional wellbeing, including understanding of Aboriginal family structure and culture.
* Child Protection Guidelines: Understanding of child protection guidelines within NSW Health.
* Autonomy and Teamwork: Ability to work autonomously and as part of a multidisciplinary team; prioritise own workload.
* Communication Skills: Effective written and verbal communication skills and basic computer skills.
* Driver's License: Current NSW driver license (P2) and willingness to drive for work purposes.
