About The Role

Are you ready to make a real difference in the lives of older people in your community? Narrabri Community Health Service is thrilled to welcome an enthusiastic Enrolled Nurse to join our Transitional Aged Care Program (TACP) and Community Nursing teams.

No two days will ever be the same. In TACP, you'll be out and about visiting clients in their homes—assisting and assessing personal care needs, lending a hand with light domestic duties, facilitating social support, and helping clients get to important appointments. You'll then have the opportunity to broaden your skills by working with our Community Nursing team, where every day brings new opportunities to learn and grow.

We're looking for a motivated, caring individual who is passionate about working with older people. A current NSW driver's licence and willingness to travel are essential, as you'll be visiting clients across the community. Your ability to recognise changes in a patient's condition, act swiftly in emergencies, and work with other health services will set you apart. An interest or experience in wound management is an added bonus. You'll be fully supported by our Registered Nurses, Nurse Unit Manager, and Community Health Service Manager, with orientation, mentorship, and a Professional Development Plan offering regular education and career growth opportunities.

Responsibilities

* Provide visiting services to clients in their homes as part of TACP and Community Nursing teams
* Assist with personal care, light domestic duties, and social support
* Support clients to attend important appointments
* Collaborate with Registered Nurses and other health professionals
* Recognise changes in patient condition and respond appropriately
* Travel across the community to visit clients (requires NSW Driver's Licence)


Requirements

* Current Authority to Practice as an Enrolled Nurse with AHPRA
* Current NSW Driver's Licence and willingness to travel for work
