About the opportunity:

We are seeking a passionate and culturally aware Registered Nurse to join our dynamic community-based team. This role is grounded in meaningful engagement with Aboriginal people, their families, and communities. You'll work in a variety of settings—from supporting local Aboriginal Medical Services (AMS) like Awabakal, to providing in-home visits, and assisting clients with GP, outpatient, and telehealth appointments including those with Dr Oakley. You may find yourself providing care in homes, clinics, or through telehealth, guided by a client-centred, flexible, and non-judgmental approach.

Each day is different, offering a unique chance to make a real difference through:

Comprehensive clinical assessments

Health coaching and education

Case coordination and support navigating services

Health screening and 48-hour follow-up phone calls

Yarning sessions to promote health literacy

You'll be supported by a tight-knit team of Registered Nurses, a Clinical Nurse Consultant, and culturally informed community service teams. For the successful candidate there is a comprehensive orientation package with ongoing professional and cultural support.

About you:
You are a Registered Nurse with experience or a strong interest in chronic disease management, particularly in areas such as diabetes, cardiac or renal care. You may also bring skills in case management, and a passion for improving long-term health outcomes in the community.

Essential Requirements:

Current Authority to Practice as a Registered Nurse with AHPRA

Eligible to drive in NSW with a current unrestricted NSW Drivers Licence
