We are seeking a passionate and experienced Elder Care Team Leader to support Aboriginal and Torres Strait Islander Elders in navigating the aged care system with confidence, dignity and cultural safety. This role is central to ensuring Elders and their families receive the right support, understand their entitlements, and are empowered throughout the aged care assessment and service journey.
About the Role
The Elder Care Team Leader works closely with clients, families, aged care assessors and service providers to ensure Elders:
Access appropriate local aged care services
Receive culturally safe plan assessments
Understand My Aged Care registration, entitlements, contracts and care costs
Are well prepared for assessments, plan reviews and outcomes
Have advocacy support to ensure care and cultural needs are met
The role works collaboratively with Elder Care Connectors and other health staff and may vary depending on community needs.
Key Responsibilities
Provide pre-assessment support, including identifying services and access pathways
Support clients during the aged care assessment process by briefing assessors and participate in assessment interviews where required
Collaborate on development and review of support plans, explain contractual arrangements and costs of care
Assist clients and families to select appropriate service providers and support family members with care transitions and related services
Advocate to ensure cultural and care needs are met
Build knowledge of local aged care and community services and maintain strong relationships with providers and community networks
Engage with My Aged Care and local assessment teams
Coordinate and deliver health promotion activities for Elders and families
Provide education and develop accessible, culturally appropriate health resources
Support Elders' participation in community wellbeing activities
Participate in Continuous Quality Improvement (CQI) activities
Essential Experience & Skills
Experience working with Aboriginal and Torres Strait Islander people
Strong communication skills with the ability to build community trust
Understanding of person-centred and trauma-informed practice
Knowledge of My Aged Care and aged care operational guidelines
Experience supporting clients to navigate complex service systems
Strong record management and reporting capability
Ability to work autonomously and collaboratively
Established community and service sector networks
Qualifications & Requirements
Qualification in a health or care-related field (desirable)
Non-adverse National Criminal History Check
Current OCHRE Clearance
Acceptable vaccination status for working with vulnerable people in the NT
Right to work in Australia
