Aged Care Comprehensive Assessor

Key Responsibilities:

* We are seeking a skilled Aged Care Comprehensive Assessor to join our team. This role plays a vital part in identifying the needs of older adults and connecting them with services that support independent living.

Primary Responsibilities:

* To conduct thorough assessments of older adults, identifying their needs and developing personalized plans to support their well-being.
* Facilitate referrals to relevant services, ensuring seamless connections for our clients.
* Promote the retention and maximization of function and capability in daily living, empowering older adults to maintain their independence.

Fundamental Skills:

* Thorough understanding of Department of Health contractual requirements.
* Excellent communication and interpersonal skills, with the ability to build strong relationships with families and carers.
* Ability to work closely with multidisciplinary teams, promoting a collaborative approach to care delivery.
