Job Description
This position can be based in either our Butler or Mosman Park office, providing customer-based services involving the admission and assessment of consumers in line with their needs and goals. By partnering and working with consumers we assist in enabling them to safely live in their own home and community.
The successfulapplicant will be someone who can independently manage and grow a portfolio of Support at Home clients.
Key Responsibilities include:
* Partnering with consumers to develop care plans
* Ongoing case management of existing Support at Home consumers (previously HCP)
* Proactively promote White Oak and Care Net to potential consumers and community referral partners (GPs, Social Workers, Assessment Agencies, Hospitals, Pharmacies etc.)
* Regular monitoring and monthly reporting on consumer budgets
* Working closely with White Oak and Care Net Clinical and Allied teams
* Scheduling appointments for assessments and reviews
* Working with other program and scheduling Managers to ensure a consistently high level of service is delivered
* Keeping up-to-date with government legislated standards, guidelines and reforms

* Previous experience in Home Care Package and/or Support at Homecoordination
* Highly developed communication and organisational skills
* Ability to work in a dynamic, fast-paced and changing environment
* High level computer literacy (MS Office applications and CMS; preferably Alayacare)
* Kind, caring and respectful of consumers and colleagues identity, culture and diversity.
