Clinical Nurse Consultants play a pivotal role in clinical service, consultancy, leadership, research, education, and services planning and management.
The ideal candidate will be responsible for leading multidisciplinary meetings to review patients with extended hospital stays, identifying barriers to timely discharge. They will also monitor and report on clinical performance metrics, implementing improvement strategies. Additionally, they will develop complex discharge plans ensuring safe transitions from hospital to home or other care settings.

The selected candidate will be responsible for facilitating family meetings to discuss care plans, concerns, and decision-making regarding discharge. They will also coordinate interfacility transfers maintaining patient safety and continuity of care.
Key Responsibilities
1. Lead multidisciplinary meetings to review patients with extended hospital stays, identifying barriers to timely discharge.
2. Monitor and report on clinical performance metrics, implementing improvement strategies.
3. Develop complex discharge plans ensuring safe transitions from hospital to home or other care settings.
4. Facilitate family meetings to discuss care plans, concerns, and decision-making regarding discharge.
5. Coordinate interfacility transfers maintaining patient safety and continuity of care.
Requirements
* Registered with AHPRA as a Registered Nurse.
* Clinical Nurse Consultant/Clinical Midwife Consultant Grade 2 with at least 5 years post-registration experience, preferably in an acute setting, with approved postgraduate nursing qualifications or equivalent experience.
* Demonstrated ability to apply clinical expertise in a consultant role and work independently.
* Proven clinical leadership and ability to collaborate within a multidisciplinary framework with commitment to excellence in nursing practice.
* Evidence of critical thinking related to advanced evidence-based nursing care.
* Experience leading and implementing change, translating evidence-based research into practice within a person-centred framework.
* Strong interpersonal, written, and verbal communication skills, and advanced computer skills for data analysis and reporting.
* Ability to contribute to strategic and clinical service planning with experience in reducing length of stay and navigating complex discharges and family dynamics.
