The Team

The Tasmanian Health Service delivers the Transition Care Program (TCP), a short-term community-based service supporting older Tasmanians after a hospital stay. Our multidisciplinary team provides person-centred care to help clients regain independence and achieve recovery goals. The TCP team partners with key stakeholders to deliver healthcare excellence.

The Role:

The Clinical Nurse Specialist (CNS) plays a vital role in the TCP, overseeing referral triage, care planning, and coordination within a multidisciplinary team. The CNS also ensures compliance with legislation and leads quality improvement initiatives.

You will need:

* Advanced clinical nursing knowledge and skills, with experience in evidence-based primary healthcare for older adults or those with cognitive impairment.
* Leadership ability to collaborate with the TCP Manager in staff and resource planning and coordination.
* Ability to work autonomously and within a team to deliver effective client care and discharge planning, utilizing various information technologies.

Details of appointment:

Permanent, full-time position, working 76 hours per fortnight, commencing as soon as possible. Hours are negotiable.

Applicants should note that this process may be used to fill similar vacancies for up to twelve months from the publication date.

Salary: $106,570 to $112,440 per annum, with an additional 12% superannuation contribution.

* Salary in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

Postgraduate allowance of up to 10% based on qualifications may apply.

Salary Packaging:

Eligibility:

Must be registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.

Desirable: Postgraduate qualifications or working towards them.
