Support Coordinator – Join the Summerland TeamSCHADS Award Level 4
Summerland Support Services is currently seeking an experienced and well-organised Support Coordinator to join our team on a long-term basis. You'll play a key role in delivering exceptional, tailored support to meet the diverse needs of our NDIS participants.
Key Responsibilities
Provide support coordination and/or psychosocial recovery coaching to NDIS participants with varying support needs
Use a person centred approach to enhance participants’ capacity, choice, and control
Collaborate with clients to develop, implement, and monitor individualised support plans
Identify and address complex challenges, barriers, and risks alongside stakeholders
Demonstrate a solution-focused mindset and encourage innovative thinking
Contribute to organisational development and local community networking
Adapt to evolving participant needs and circumstances
Foster a welcoming, inclusive, and respectful environment for people from diverse backgrounds
Required Qualifications & Experience
Minimum 2 years’ experience in NDIS Support Coordination or a similar role
Minimum Cert IV in Community Services, Disability, or a related health discipline
Strong knowledge of NDIS operational guidelines
Current driver’s licence and access to a safe, comprehensively insured vehicle
NDIS Worker Screening Check and Working with Children Check
Completion of the NDIS Worker Orientation Module
First Aid and CPR certifications
Excellent interpersonal and communication skills
Strong computer literacy, including proficiency in Microsoft Office
Ability to work independently and proactively
