Responsibilities
In this role you will assess individual client needs, plan and coordinate care and provide clinical guidance to less experienced staff.
* Deliver proficient, person‐centred palliative nursing care and case management across multiple community locations.
* Assess, plan, implement and evaluate care to achieve the best possible outcomes for clients and families.
* Provide leadership, support and guidance to Enrolled Nurses, students and less experienced staff.
* Contribute to a culture of continuous learning, collaboration and quality improvement within the palliative care team.
* Support clients to remain at home for as long as possible.
Qualifications & Requirements
* Passionate about providing compassionate, holistic palliative care that supports clients to live with dignity and comfort.
* Experienced and confident nurse who thrives in community‐based care and enjoys working across diverse settings.
* Strong communication and problem‐solving skills that allow you to connect with clients, families and colleagues alike.
* Looking for a flexible role where you can share your expertise, mentor others and continue to grow professionally.
* Must be immunised and able to undergo employment screening.
* Must hold registration with AHPRA to practice as a Registered Nurse within the organisation.
