As part of the Statewide Hospital Pharmacy Northwest team, you will be responsible for providing comprehensive clinical pharmacy services to patients across various stages of care.
* Clinical Pharmacy Services:
o Best Possible Medication History taking and development of Medication Management Action Plans in collaboration with multidisciplinary teams.
o Patient education throughout the inpatient care journey.
* Medication Dispensing:
o Dispense medications according to legal requirements and within the parameters of medication access schemes.
* Meds Advice:
o Offer verbal and written medicines information and health advice to patients.
