Role Description
The Social Worker will provide direct support and advocacy to clients by assessing their needs, creating care plans, and connecting them to community resources. Day-to-day responsibilities include conducting home visits, maintaining accurate records, delivering therapeutic interventions, and collaborating with other health professionals to ensure holistic care. This is a full-time, on-site role located in Hobart, TAS.
Qualifications
* Client assessment, case management, and care plan development skills
* Knowledge of community resources and social services networks
* Strong interpersonal communication and advocacy skills
* Proficiency in maintaining accurate and confidential documentation
* Experience in therapeutic interventions and counseling practices
* Degree in Social Work or equivalent qualification
* Current registration with a recognized Social Work governing body
* Ability to work collaboratively as part of a multidisciplinary team
* Experience in aged care, mental health, or disability services is an advantage
