Occupational Therapist Senior (Frailty Virtual) (Bundaberg)
Wide Bay Hospital and Health Service is looking for an experienced Occupational Therapist to join our dynamic team at Bundaberg Hospital, providing advanced clinical care to older adults experiencing frailty through a virtual and home-based model of care. The role requires leading and mentoring staff, contributing to service development, and delivering person-centred care in a supportive workplace.
Base pay range: A$89,718.00/yr - A$110,152.00/yr
Responsibilities
* Deliver high level occupational therapy clinical services and professional expertise to patients identified for the rapid access geriatrician-led model of care.
* Work autonomously within the multidisciplinary team to manage, coordinate and provide optimal, responsive and flexible Occupational Therapy services in acute, community, and virtual settings.
* Focus on preventing inappropriate or avoidable admissions by supporting the needs of the patient, their family, and significant others through sub-acute supports in the home environment.
* Provide short sub-acute service (up to 14 days) to deliver intervention in the home environment to reduce the need for hospitalisation and facilitate reablement through improvements to function in the home environment.
Qualifications & Experience
* Demonstrated ability to provide high level Occupational Therapy knowledge and skills with complex work that requires expansion, adaptation or modification, with infrequent need for direct supervision.
* Extensive knowledge of medical, surgical, and cognitive conditions impacting functional ability and ability to use this knowledge to help patients remain safely in their own homes.
* Strong interpersonal, negotiation and communication skills for effective verbal and written communication within a multidisciplinary team.
* Effective time management skills to prioritise clients across acute and community settings.
* Experience providing clinical and operational supervision and support to less experienced staff.
* Broad understanding of the continuum of care from acute inpatient to community setting, including government and non-government agencies and services within the Bundaberg Health Service.
Preferred Qualifications
* Tertiary degree (or equivalent) in Occupational Therapy and registration with the Allied Health Professional Registrations Agency (AHPRA) or relevant allied health registration board.
* Proof of qualification and registration or membership with the appropriate authority, presented to the supervisor/manager prior to commencement of duties.
* Additional relevant qualification such as cognitive impairment related training and Functional Independence Measure (FIM) training is well regarded.
