About the Role An opportunity to join St Vincent’s Hospital Melbourne in a Grade 2 Occupational Therapist role within the Transition Care Team. The successful applicant will provide high quality occupational therapy services; supporting assessment, intervention and discharge planning for clients on TCP. You will work within a highly skilled multidisciplinary team to deliver outcomes that are determined by client care plans and goals. The role is responsible for the supervision of students and allied health assistants. Applications from current grade two occupational therapists or suitably experienced grade one occupational therapists will be considered. Relevant postgraduate qualifications would also be highly regarded. Duties: · Provide high quality and optimal occupational therapy assessment and clinical management of Transition Care Program clients. · Plan, implement and monitor appropriate treatment programs from established client goals using best practice principles and evidence-based practice. · Provide carer/family support to assist in the management of Transition Care Program clients at home.
