About the Role
We are seeking a skilled Care Partner to join our team at a leading healthcare provider.
As a Care Partner, you will play a vital role in supporting clients to remain safe, independent, and connected at home.
* Assessment & Care Planning: Conduct comprehensive intake assessments; develop and review goal-directed care plans using a rights-based approach.
* Care Coordination: Coordinate services to support client goals; maintain regular monitoring; manage changes in needs or risk; liaise with families, GPs, allied health professionals, hospitals, and other providers.
* Safety & Compliance: Ensure practice complies with relevant laws and regulations; manage risk, incidents, SIRS reporting, and uphold privacy, dignity, and boundaries.
