The Opportunity
We're looking for a passionate, qualified Diabetes Educator to join our dedicated team. Reporting to the Chronic Care Manager, you will provide visiting services across Sunrise communities, delivering individual and group education and care to people living with, or at risk of, diabetes. This is your chance to make a tangible difference in the lives of Aboriginal families while enjoying the adventure and unique culture of the Big Rivers region.
Key Responsibilities
* Deliver high‑quality, individualised diabetes education and management across Sunrise communities
* Work in partnership with health centre and specialist staff to coordinate care
* Provide clinical leadership and support to RANs, Aboriginal Health Practitioners, and other health professionals
* Promote a multidisciplinary approach to chronic disease management
* Manage MBS claims where applicable
What You'll Bring
* Current registration with AHPRA as a qualified health professional
* Accredited Diabetes Educator (or eligibility to obtain credentialing within 12 months) with demonstrated experience in diabetes and chronic disease management
* Strong communication and interpersonal skills
* Capacity to work independently and manage a mobile workload
* National Police Clearance and OCHRE Card (or ability to obtain)
