About the Role

EIPSR provides specialist psychosocial support to help eligible participants to build their capacity to better manage their mental health, develop practical life skills for independent living and social connectedness, and if eligible, transition to the NDIS.

* Conduct screenings and assessments, and participate in case planning and exit reviews alongside care teams and clinical staff.
* Advocate for individuals and build sustainable support networks.

This role works on a hybrid system, including working from home, in our Geelong office and assertive outreach support to participants across the Greater Geelong region and Bellarine Peninsular.
