The Opportunity
Working closely with the Facility Manager, you will lead all aspects of Clinical Management and care delivery across the home.
Key areas of responsibility include:
* Clinical leadership of RN and care teams
* AN-ACC funding optimisation and documentation oversight
* Accreditation compliance and quality standards
* Continuous improvement initiatives
* Medication management and infection prevention control
* Clinical audits, indicators and performance monitoring
* Incident management and risk mitigation
* Care planning, assessments and case management
* Family communication and case conferences
* Acting FM support when required
This role requires someone confident in managing complexity, driving accountability and leading a multidisciplinary team.
Who We're Looking For
We are open to:
* An experienced Care Manager ready to lead with confidence, or
* A strong Clinical Care Coordinator / Senior Registered Nurse ready to step up into a broader leadership role
You will demonstrate:
* Current AHPRA registration as a Registered Nurse
* Experience within residential aged care
* Strong knowledge of AN-ACC funding and accreditation standards
* Experience in clinical audits and quality improvement
* Confidence supervising and mentoring nursing teams
* Strong documentation and electronic care system skills
* Sound clinical capability across wound care, palliative care and chronic disease management
