About the Role The Case Manager will manage caseloads of participants with complex needs, lead a team of Disability Support Workers, and ensure operational excellence across rostering, claims, approvals, and compliance. Reporting to the Senior Case Manager, this role is operational and hands-on, with occasional direct participant support, travel across SA, and a focus on maintaining strong relationships with participants, families, guardians, and support coordinators. What You’ll Do Lead a team of Disability Support Workers with complex psychosocial, intellectual, and aging-related disabilities. Oversee rostering, approvals, claims, and operational administration. Provide hands-on participant support as required, based on complexity. Maintain strong stakeholder relationships and attend meet & greets. Ensure services meet NDIS and/or Aged Care compliance standards. Support the Senior Case Manager in stabilising services and operational delivery. About You Certificate IV or higher in Disability, Aged Care, Community Services, Nursing, or related field. Minimum 2 years’ experience in case management, in-home care, or support coordination. Strong administrative and operational skills: rostering, claims, approvals, payroll. Experience with NDIS participants and systems. Excellent communication and stakeholder engagement skills. Hands-on participant support experience; ability to travel to client homes. Current First Aid, CPR, NDIS Worker Screening Check, driver’s licence, and access to a reliable vehicle.
