OverviewPart Time, Fixed Term Contract - 28/01/202632 hours per fortnightNortheast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.NHW have an exciting opportunity for a passionate and experienced staff member to join our team as a Sub-acute Ambulatory Care (SACS) Co-ordinator.In this vital role, you will provide day-to-day operational leadership and coordination across the SACS and Transition Care Program (TCP) services.
The role is key to delivering high-quality, goal-directed care that supports clients' independence, promotes hospital admission avoidance, and ensures smooth patient flow throughout Northeast Health Wangaratta (NHW).
