About the Role:
This position is responsible for providing clinical social work services as part of a multidisciplinary team. You will work with patients across various settings, including hospitals and community facilities, providing complex case management, psychosocial assessments, therapy interventions, and monitoring high-quality social work services.
Key Responsibilities:
* Conduct psychosocial assessments and develop treatment plans to address the unique needs of patients.
* Deliver therapy interventions, including individual and group sessions, to promote patient well-being and resilience.
* Mobilize services and supports to ensure patients receive comprehensive care and support.
Requirements:
* A Qualification degree in Social Work from a recognized tertiary institution.
* Professional membership with the Australian Association of Social Workers (AASW).
