Respite CoordinatorCalvary - Flinders Far North (Pt Augusta)Full Time - Permanent Role About the role We are seeking an enthusiastic and motivated Client Coordinator to join the Operations team in the Flinders Far North Service Centre, overseeing and working at our Port Augusta Respite Cottage.As a Respite Coordinator, you will ensure the services being provided to care for approved Community Home Support Program participants and their carers is provided in a professional and caring manner consistent with Calvary's Mission, Vision and Values and in line with current legislation and Standards. In this position, you will be responsible for assessment and coordination of responsive and flexible quality care that promotes care aligning with participants choice and person cantered approach.In this role the Client Coordinator will be accountable for:Delivering the best support and quality service for Community Home support participantsIncreasing and monitoring of service delivery hours to meet KPI expectationsWorking under Aged Care standards, Department regulations, Calvary policies & proceduresEnsuring participant reviews and care plans are up-to-date and completed within KPI timeframesEscalating Incidents, complaints, feedback, directly to into RISKMAN and escalating to the Service Centre ManagerCompleting CHSP service agreements and budget tools with the participantIdentifying deterioration and continuous liaison with health professionals and other agenciesArranging excursions/ outings in consultation of client's choices and interestsDay program planning and evaluations of programming to meet individual client care needsOverseeing meal planning ensuring dietary requirements are met Applicants are required to meet the following criteria:Demonstrated critical thinking and collaborative practiceResults driven with a demonstrated ability to empower othersDemonstrated ability to prioritize workload and meet deadlinesStrong communication, interpersonal, organization and ethical decision making skillsAbility to build positive relationships with participants, their families and support workersThe ability to work autonomously and as part of a teamDemonstrated commitment to excellence in customer service and continuous quality improvement.Concise and accurate documentation skills with excellent time management abilitiesProficient in Computer -Excel, word, outlook and systemsWorking hours of Mon- Friday 9:00am - 5:00pm Why work for Calvary?
