About the Role:
These positions offer a flexible work schedule with rotating roster options, allowing for a variety of personal commitments and lifestyles. You will work days from Monday to Sunday on a rotating roster.
Key Responsibilities:
* Provide support to improve independence by assisting with living skills, transport, social activities, medication, and personal care.
* Maintain customer records up to date using online systems.
* Develop and maintain professional relationships with people, families, caregivers, and key stakeholders.
* Assist in the development or implementation of care plans.

Requirements:
* A certificate-level qualification in Disability or Community Services/Individual Support or equivalent relevant experience in the human services industry.
* A valid NSW Drivers C class licence, with the ability to drive large cars and vans.
* You must be fit to undertake the physical requirements of customer support, including lifting up to 10 kilograms unassisted.
* Intermediate skills in Microsoft Office and other common systems.
Desirable:
* Experience in disabilities, mental health conditions.
* Advanced knowledge of services and programs under NDIS.
* Understanding of Person-Centered Practices.
