*The Role
An opportunity has become available for a Social Worker (Mental Health - Homeless Health Outreach Team) to join Gold Coast Health (GCH) where the focus of our strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
Deliver a range of Social Work services in the consumers' environment, including bio-psychosocial assessment and planning, and implementing interventions appropriate to consumers with complex and varied health needs
Deliver a range of Generic Mental Health services including but not exclusively Mental Health assessment, case management and Outreach services to people who are experiencing homelessness
Undertake Social Work tasks within established clinical pathways, protocols and work unit guidelines
Work collaboratively with non-government (such as emergency relief agencies, emergency accommodation agencies, housing agencies, support agencies and counselling agencies) and other government agencies
About you
To be successful in this position you will require:
A tertiary degree in Social Work
Eligible for membership to the Australian Association of Social Workers (AASW)
Driver licence
Blue card / Working with children check (GCH will cover cost)
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Declaration of serious discipline history must be disclosed
Criminal History Screening will be required for new employees (GCH will cover cost)

The Homeless Health Outreach Team is a work unit within the Mental Health and Specialist Services directorate and aims to provide comprehensive assessments, case management, co-ordination and clinical interventions for homeless persons in the community who are experiencing mental illness.
