With several funding streams, Black Swan Health delivers a range of primary health services including (but not limited to), diabetes and multidisciplinary chronic conditions services, psychology and counselling services, youth mental health services, community early psychosis treatment and recovery programs, street doctor services, disability and psychosocial services.

The Role

The Assistant Manager will be responsible for supporting, supervising, and guiding the Support Coordinator Team.

Additionally, providing support and supervision for the Intake team to conduct intake assessments for potential NDIS and Psychosocial clients, and allocation of clients across the Disability and Psychosocial Services team.

The Assistant Manager will apply a person-centred and solution-focused approach to team supervision and service delivery that aligns with the implementation of the NDIS Quality and Safeguarding Framework and the National Mental Health Standards.

Duties and Responsibilities of the Position

§ Maintain a robust understanding of the NDIS and its impact on clients and continually improve processes for supporting clients through NDIS processes.

§ Support the Disability Services Manager by responding to the day-to-day supervision requirements of the Disability Services team.

§ Develop and maintain relationships with key stakeholders (including networking and outreach)

§ Review and monitor service provision to ensure continuity of engagement with service providers. Maintain and create new linkages and build relationships with relevant agencies, e.g., LAC providers, to optimise client referrals to Black Swan Health.

§ Undertake and supervise triage and allocation of clients

§ Building capacity and ensuring mechanisms are in place to allow the team to share knowledge and resources effectively.

§ Confer regularly with staff and Manager to review employee relations climate, specific problem areas and actions necessary for improvement.

§ Maintain a strong sense of client service and ability to respond to people experiencing multiple difficulties in a person-centered, calm and professional manner.

§ Undertake training as required (e.g. Supervision training)

§ Facilitate client referrals into Black Swan Health Disability Service, ensuring a high client retention rate.

§ Ensure responsive collection of data for NDIS invoicing, monitoring and reporting

§ Monitor service outcomes and provide reports to Black Swan Health, scheduled and ad-hoc.

§ Manage client information management system, ensuring timely reporting and invoicing activity and monitoring use of client support coordination hours.

§ Conduct all activities in compliance with relevant legislation, including Equal Employment

§ Opportunity, Occupational Health and Safety, and all policies, procedures and guidelines as determined by Black Swan Health.

§ Other duties as required.

The successful candidate will have

§ Current National Police Clearance

§ Working with Children's Check

§ NDIS Worker Screening Check

§ Current driver's licence and own vehicle for work purposes

§ Eligibility to work in Australia

§ High-level computer skills, including demonstrated experience with Windows-based packages and client information management systems.

§ Ability to work as part of a team, as well as be self-motivated to work independently.

§ Ability to think conceptually, analyse issues and develop innovative solutions and strategies within organisational and service system constraints.

§ Experience in direct client service delivery within the mental health or disability sectors.

§ Experience managing and coordinating multidisciplinary teams

§ Demonstrated knowledge of National Disability Insurance Scheme service delivery requirements

§ Excellent leadership skills in complex systems and proven experience in program service coordination

§ Demonstrated experience working with people who live with a disability and/or with severe and persistent mental illness.

§ A demonstrated person-centered approach.

§ Strong case and stakeholder management skills.

§ Ability to work autonomously but as part of a multi-disciplinary team across diverse locations.

§ Highly developed interpersonal communication skills, including relationship building, negotiation and conflict resolution skills

§ Relevant Degree level qualification in Social/Human Sciences, including Psychology, Social Work, Counselling
