Bayside Health Bayside Health is a public health service delivering high-quality care across every stage of life for close to 1.2 million people living in metropolitan Melbourne, the Mornington Peninsula, Koo Wee Rup, Bass Coast and Southern Gippsland. We have more than 15 main sites, including hospitals, centres and clinics that provide comprehensive care from welcoming newborns to supporting older people and a full range of services in between. Responsibilities
* Provide social work services across a broad clinical caseload within the health service in a patient-centred, multidisciplinary environment.
* Manage a clinical caseload of varying complexity; conduct psychosocial assessment and intervention.
* Support discharge planning and care coordination; contribute to patient flow and service delivery outcomes.
* Participate in supervision, teaching and learning activities; support quality improvement and evidence-based practice initiatives.
* Engage with patients, families and carers to address psychosocial impacts of illness and hospitalisation, including counselling, emotional support and advocacy.
Grade and Qualifications
Grade 1
* Demonstrated ability to provide social work services within a hospital setting.
* Developing skills in assessment, intervention and discharge planning.
* Ability to work effectively within a multidisciplinary team.
* Strong communication and interpersonal skills.
Grade 2
* Demonstrated experience in delivering social work services in a hospital setting, with a focus on cancer care.
* Ability to manage a clinical caseload, including patients with more complex needs.
* Well-developed assessment, intervention and care coordination skills.
* Ability to exercise professional judgement and work with increasing autonomy.
* Experience contributing to supervision, teaching or quality activities (desirable).
Requirements for All Applicants
* Demonstrated hospital-based experience is an essential requirement of this role.
* Bachelor of Social Work (or equivalent).
* Eligibility for membership with the Australian Association of Social Workers (AASW).
