Current registration with AHPRA and credentialing in Mental Health Nursing.

Current registration with AASW or eligibility for membership with the Australian Association of Social Workers (AASW) as an Accredited Mental Health Social Worker.

Base Pay Rate (Annual):
AHP Level 2: $ Pro Rata + Super + Leave Loading + Salary Packaging)

About the Role
We are seeking a passionate and skilled Pathways to Recovery Clinician to join our multidisciplinary team supporting individuals with severe and complex mental health needs as they transition from tertiary services to community-based care.
This role is ideal for someone with experience in delivering trauma-informed, recovery-oriented mental health services, and who thrives in collaborative environments. You will be responsible for conducting comprehensive assessments, developing recovery care plans, delivering evidence-based therapeutic mental health interventions, and coordinating care with GPs, Local Health Networks, and psychosocial services. You'll also work closely alongside Peer Support Workers to ensure clients and their families are supported throughout their recovery journey.
The position includes intake, triage, therapeutic intervention, care coordination, and active participation in clinical review and service improvement processes. Services are delivered across FocusOne Health's catchment areas, including Berri, Mount Gambier, and Murray Bridge.
Candidates with qualifications in Social Work, Occupational Therapy, Mental Health Nursing, Registered Nursing, or Psychology, and who hold current registration with AHPRA or relevant professional bodies, are strongly encouraged to apply.

The successful applicant will be remunerated in accordance with FocusOne Health's Enterprise Bargaining Agreement (EBA) dependent on skills, experience, and qualifications.
