Service Coordinator – Disability Services
Location: Primarily work from home with regular travel required for site visits. Candidate must be based within a reasonable travel distance from Toowoomba.

About the Role
The Service Coordinator is a vital operational role, responsible for ensuring the seamless delivery of supports to participants. Acting as the key link between participants, families, support workers, and stakeholders, the SC oversees day-to-day service delivery, provides leadership to frontline staff, and ensures supports are aligned with participant goals, NDIS plans, and organisational standards.
You will:
· Oversee and coordinate participant supports, ensuring quality and consistency.
· Act as the primary point of contact for participants, families, and stakeholders.
· Supervise, and mentor Support Workers, fostering a positive team culture.
· Facilitate smooth onboarding of new participants and staff.
· Ensure compliance with NDIS Practice Standards, organisational policies, and reporting requirements.
· Respond to emerging service needs and participate in the on-call roster.
About You
We are looking for a motivated and compassionate leader who can balance operational priorities with a genuine commitment to participant outcomes. You will be organised, proactive, and confident in managing relationships with a wide range of people.
Essential:
· Certificate IV or higher in Disability, Community Services, Social Work, Psychology, or related field.
· Demonstrated experience in disability, community, aged care, or home care services.
· Strong leadership skills with the ability to support and motivate frontline staff.
· Excellent communication and interpersonal skills.
· High level of organisation, problem-solving, and conflict resolution abilities.
· Proficiency in Microsoft Office and ability to learn new systems.
· Current Driver Licence, NDIS Worker Screening Check, Blue Card, First Aid & CPR.
Desirable:
· Diploma or Bachelor's qualification in a related field.
· Experience with NDIS service coordination and participant management systems (Visual Care advantageous).
· Knowledge of the SCHADS Award and rostering environments.
