The Position Evaluate, reconcile and plan for patients’ optimal and safe medication management pathway across the continuum of care from admission to discharge. Monitor the patients’ total medication regimen to promote safe, rational and cost effective medication management showing due regard to admission medication history and reconciliation, medication alert status, pathology test results, drug levels, interactions, adverse reactions, medication effect, side effects, toxicity, dosage, formulation, compliance in administration and costs. Medication chart management ensuring orders are clear and appropriately prescribed. Ensure that the prescribing of all medication conforms to legal, hospital and Pharmaceutical Benefits Scheme requirements and that such prescribing is clearly understood by all personnel. Liaise with medical, nursing and other health professionals, to provide medication information, and promote rational cost-effective therapy. Counsel patients on their medication when appropriate. Organise and dispense appropriate medications, compliance aids, including computer generated Consumer Medicine Information. Liaise with the purchasing and dispensary team about medication orders. Develop and participate in education programs for pharmacists, pre-registration pharmacists, 4th year pharmacy students, nursing, medical and other allied health caregivers. Supervise 4th year pharmacy students and pre-registration pharmacy interns as appropriate. Assist with the supervision of Pharmacy Technicians assigned to the area. Attend and participate in departmental meetings and continuing education meetings. Understand and interpret the PBS including Section 100 drugs and private health insurance issues. Understand and interpret relevant Section 100 and SAS legislation including access, supply, monitoring and documentation. Record all movement of Schedule 8 and Schedule 4 Restricted medications in Drug Registers consistent with relevant State Government Legislation. Identify areas for quality improvement and report medication related issues to the Pharmacy Coordinator (Clinical Services) Have an appropriate knowledge of equal employment opportunity and occupational safety and health principles pertinent to the position. Participate in a weekend roster for clinical services. Undertake any duties assigned by the Pharmacy Coordinator (Clinical Services), other Pharmacy Coordinators or Chief Pharmacist commensurate with the position of a clinical pharmacist. You will hold current registration with the Pharmacy Board of Australia and have a demonstrated involvement in continuing education and training programs and a commitment to continual professional development. You will possess the knowledge necessary to provide clinical pharmacy services to patients and other health professionals in a hospital environment.
