Community Health Professional Opportunity
We are seeking a skilled Community Health Professional to join our team. In this role, you will work with a person-centred approach to deliver responsive and high-intensity intermediate intervention to clients who otherwise may require Emergency Department attendance and/or hospitalisation.
Key Responsibilities:
* Collaborate with Clinical Nurse Specialists and Nurse Practitioners to provide optimal healthcare to clients in the community
* Utilize excellent clinical assessment skills to inform care decisions
This is a permanent, full-time position, working up to 76 hours per fortnight. Commencement as soon as possible is required.
Remuneration Package:
A competitive salary of $78,179 to $105,995 per annum (Pro rata) plus a generous Employer superannuation contribution of 12%.
Successful applicants for this role will have access to career progression opportunities within the organisation, including advancement to Grade 4.
