The Opportunity

As the Dementia Care Coordinator, you will lead with purpose, shaping high quality dementia care that empowers residents, supports families, and inspires staff as a key member of our growing Quality, Risk & Assurance team.
This Full-Time onsite role is a unique opportunity to utilise your clinical & practice expertise, compassionate leadership, and a commitment to delivering person-centred dementia care to support dignity, independence, and quality of life. This role is available for a temporary contract of minimum 1 year.

Responsibilities:

* Leading the delivery of dementia care services across Carrington Care
* Developing and implementing individualised, person-centred care plans & conducting comprehensive assessments
* Drive best practice dementia care processes, supporting and educating staff in dementia care best practices
* Collaborating with multidisciplinary teams and external specialists
* Fostering a dementia-friendly environment and culture
* Engaging families, providing emotional support, and facilitating support groups
Driving innovation through research, quality improvement, and policy development

About you:
Essential

* Registered Nurse, with other relevant qualifications desirable (e.g. Behaviour Support Practitioner, Allied Health Professional)
* Minimum 2–3 years' experience with supporting dementia or other functional cognitive disorders
* Expertise in dementia care principles and behavioural strategies
* Experience in clinical governance & risk activities
* Experience in clinical leadership, mentoring and training staff
* Passion for quality care & continuous improvement
* Strong communication and leadership skills
NDIS Worker Check (or willing to obtain)
