Senior Care Coordinator

Bethanie, a leading not-for-profit aged care and retirement living provider, warmly welcomes and cares for seniors with a rich history.

We seek a highly motivated individual to further their career in a senior care coordination position. This role is open to candidates with strong backgrounds in case management or allied health/registered nurse roles.

The senior care coordinator coordinates care for community clients within a holistic rehabilitation and wellness approach, enhancing lifestyle and living experiences for those at home. The coordinator participates in implementing client-directed models of care, empowering clients to maximise quality of life, promote healthy ageing, and facilitate community re-engagement.

Bethanie provides support, training opportunities, and tools for effective care coordinators. Ideal candidates are innovative and committed to driving excellence and quality service to our clients.

As a valued member of the team, you'll be supported by a clinical nurse, RN, EN, OT, physiotherapist, and regional manager. You'll share a caseload of 80 clients with another care coordinator and manage approximately 35-40 clients personally.

This permanent full-time position requires 38 hours of work across Monday to Friday (8am - 4pm). You may need to be on call outside of working hours and weekends as required.


About the Role:

* Client and goal-centred approach to care planning, incorporating regular reviews to ensure currency and appropriateness of care plans.
* Develops sustainable relationships with local networks and external agencies to strengthen access to social opportunities and lifestyle options.
* Monitors and manages client budgets in line with approved package levels and individual requirements.
* Provides effective support as an integral member of the multi-disciplinary team to deliver holistic care to all clients.


About You:

* A strong background in case management, allied health, or registered nursing is desirable.
* Experience managing a team of care providers.
* Excellent customer service, negotiation, communication, and support skills.
* Strong verbal and written communication skills, computer skills, and experience with client database systems.
