At Yorke and Northern Local Health Network, we believe in providing holistic care that makes a difference in people's lives. As a **Transitional Care Package - Case Manager**, you'll be part of a multidisciplinary team helping older people transition from hospital to home with confidence and dignity. You'll directly contribute to their rehabilitation and long-term care arrangements, providing essential support that allows them to regain their independence.
**Key Responsibilities**:
- Develop and implement tailored care strategies for clients under the Transitional Care Program.
- Collaborate with a diverse team of health professionals and external service providers.
- Reduce hospital stays and enhance access to services for vulnerable groups.
- Support clients through rehabilitation, ensuring successful hospital discharge and community reintegration.
- Coordinate and evaluate a range of services, from therapy and nursing to personal care.
The successful applicant will be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
