The Role of a Team Leader is instrumental in providing top-notch care to residents at all times, with a focus on delivering exceptional patient outcomes.
Key Responsibilities
* Collaborate with the care team to ensure holistic and person-centered support is provided to residents, addressing their physical, emotional, and social needs.
* Co-ordinate and oversee the implementation of resident care plans, ensuring seamless integration across departments.
* Liaise with multidisciplinary teams to facilitate effective communication and coordination of care.
* Conduct regular reviews of resident care to identify areas for improvement and implement evidence-based practice changes.
About You
* Relevant qualifications: Diploma of Enrolled Nursing (with AHPRA Registration to administer medication) and current National police check.
* High level of commitment, integrity, honesty, and trustworthiness, with excellent interpersonal and communication skills.
* Able to work effectively in a fast-paced environment, prioritising tasks and managing competing demands.
* Proficient in using information technology systems and programs, with a willingness to learn new skills as required.
* Working knowledge of WH&S and Infection Control principles.
