As a Care Manager, you will ensure the highest quality of care for clients, managing their experience and working closely with our office team to ensure seamless service delivery. Located in Osborne Park, you will advocate for clients and help achieve our business goals.

Key Responsibilities
* Planning, managing, and reviewing client goals.
* Communicating with team and clients to develop care plans.
* Formalizing agreements with clients and families regarding services and costs.
* Documenting interactions and sharing feedback about Caregiver performance.
* Meeting clients and families face-to-face to review care plans and goals.
About You
* Experience working with seniors or in aged care.
* Experience with Home Care Packages.
* Highly organized with good time management.
* Ability to work under pressure and multitask.
* Strong communication skills.
* Valid Australian Driver’s License.
* Health-related qualification (desirable).
