What you'll be doing
The Wellbeing and Health in-reach Nurse (WHIN) Coordinator CNS2 is responsible for facilitating the identification of health, wellbeing, and social needs, as well as coordinating appropriate early intervention, assessments, and referrals for school-aged children and young people attending designated primary and secondary schools.
The WHIN Coordinator CNS2 is a Registered Nurse working in local schools, applying specialist clinical nursing knowledge, experience, and skills to provide complex health care coordination for students.
This role requires leadership, specialist skills, autonomy, and the ability to work independently in an interagency context. Coordination with multiple agencies, professions, and groups is essential.
The scope of the WHIN Coordinator role includes:
* Collaborating with school principals, wellbeing teams, counselors, psychologists, staff, and local services to deliver the care model
* Building trusting relationships with students and families to identify health needs and barriers to accessing services
* Establishing a family-focused approach to connect students and families to services
* Identifying and triaging health needs
* Coordinating early interventions, assessments, and referrals
* Contributing to care coordination and case management of vulnerable children, young people, and families
* Supporting health promotion programs to improve community health and health literacy
* Working within established protocols across Health and Education sectors
* Maintaining relationships with service providers and referral pathways
* Documenting service events confidentially and accurately
Activities out of scope include:
* Providing health and wellbeing lessons in classrooms
* Conducting mental health interventions with students
* Performing population health screening (coordination of access only)
* Supporting health care procedures at school (e.g., toileting, tube feeding, tracheotomy suctioning, diabetes management)
About you, do you have?
* Current registration with the Nursing and Midwifery Board of Australia, relevant post-registration qualifications, and at least 3 years' experience working with children and young people. Qualifications in child and family health, pediatrics, adolescent/youth health nursing, or related fields are preferred.
* Experience working with children and marginalized or at-risk groups, with a focus on health promotion, prevention, and early intervention.
* Ability to engage families and address health and mental health issues, liaising effectively with health and social services.
* Experience working within interagency teams, emphasizing relationship building, service delivery, quality, and care integration.
