About Our Role
We're seeking a dedicated professional to join our team as a Home Care Coordinator. In this role, you will be responsible for conducting comprehensive assessments of client needs and developing tailored care plans that cater to their individual requirements.
Key Responsibilities:
* Conduct thorough client assessments and create personalised care plans
* Maintain open communication with clients, ensuring their needs are met and exceeded
* Cultivate strong relationships with healthcare professionals and other stakeholders to ensure seamless care delivery
To succeed in this role, you'll need the following skills and qualifications:
* Relevant qualifications (Cert IV or Diploma in Nursing highly regarded)
* Experience in aged care, NDIS, or mental health case management (preferred)
* Strong knowledge of My Aged Care, Home Care Packages, and NDIS frameworks
* Proficiency in Microsoft Office 365, SharePoint, Outlook, and rostering software
* Excellent verbal and written communication skills with strong customer service abilities
