In this rewarding role, you will coordinate holistic, person-centred supports that enable older people to remain safe, independent and connected in their own homes. Working within the new Support at Home program framework, you will undertake comprehensive assessments, develop goal-directed care plans, manage participant budgets, and collaborate with families, allied health professionals and community partners to deliver high-quality, compliant services. As a Care Partner, you will work directly with older people to assess needs, co-design personalised support plans, and monitor care delivery. This role requires knowledge of aged care principles, wellness and reablement approaches, and a strong commitment to quality and accountability as per the Aged Care Quality Standards. Key Responsibilities * Conduct initial and ongoing comprehensive assessments and develop care plans in collaboration with older adults, their families and carers. * Coordinate and monitor the delivery of services to support participant's identified needs (psychosocial, medical, physical, environmental) including personal care, domestic support, and social connection * Advocate for participants rights, preferences, choice and provide person-centre and culturally safe case coordination and care. * Establish and uphold positive working relationships with colleagues, service networks, community agencies and relevant organisations to ensure high quality provision of care that enables older adults to remain living independently in their homes. * Maintain accurate documentation in accordance with legislative and funding requirements Ideal candidates will have experience in aged care case management and have an understanding of consumer-directed care models.
