REGISTERED NURSE: COMMUNITY REHABILITATION.
Part time: 8 hours per week (ideally across 3-4 days)
We are currently seeking a passionate and motivated Registered Nurse, for the role of Community Rehabilitation Nurse.
The Rehabilitation Nurse is responsible for the provision of consumer focused holistic nursing care to those assigned to him/her, in order to achieve desired clinical outcomes. The Nurse works as part of the interdisciplinary health care team to help individuals affected by chronic illness or physical disability to adapt to their disabilities, achieve their greatest potential and work toward productive independent lives by co-ordinating and assessing designated clients within the Community Rehabilitation Centre to maximise the outcome of the client’s rehabilitation program.
What’s required?
1. Registered Nurse- Div 1and registered with AHPRA
2. Post graduate rehabilitation education and training, or willingness to complete
3. Experience in working in an interdisciplinary rehabilitation setting with a range of clientele
4. Strong clinical intake, assessment skills, program coordination and care planning within a multi-disciplinary team
5. Understanding of chronic disease management, education and promotion of self-management, health promotion and prevention principles
6. Flexibility and willingness to listen, learn, and negotiate
7. Strong personal management., Communication and organisational skills
8. Positive, motivating and supportive approach to consumer care
