About the Role: Aboriginal Health Practitioner – Aged Care
Danila Dilba Health Services delivers flexible, culturally safe Aged Care services to Aboriginal and Torres Strait Islander elders in the Yilli Reung (Greater Darwin) region. The program aims to improve access, quality, and community connection in Aged Care, empowering elders to lead the decisions about their support and services.
The Aboriginal Health Practitioner Aged Care will support a culturally safe, trauma-aware, and healing-informed service delivery aligned with the strengthened Aged Care Quality Standards and National Aboriginal and Torres Islander Flexible Aged Care program objectives. The role requires a commitment to delivering care aligned with the principles of Social and Emotional Wellbeing (SEWB), ensuring services are culturally safe, person-centred, and holistic, supporting the physical, emotional, spiritual, and cultural needs of Aboriginal and Torres Strait Islander elders through connection to land, culture, family, and community.
What You Need to Succeed
Minimum Cert IV Aboriginal and Torres Strait Islander Health Care (Practice)
* Current registration with AHPRA
* Minimum 3 years' experience working in a Primary Health Care setting.
* Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions (e.g., diabetes, cancer, respiratory and/or cardiac disease) and relevant management strategies.
* Demonstrated competence working within multidisciplinary teams or independently, with strong organisational skills and the ability to plan, manage, and prioritise tasks.
