About us:
The Older Persons Integrated Health Service (OPIHS) integrates care of the older person across the emergency department, sub-acute care, and community settings, including a rapid response team into Residential Aged Care Facilities. OPIHS is a seven-day service which incorporates the clinical streams of Geriatric Emergency Department Intervention (GEDI), Older Persons Liaison Service (OPELS), RACF In Reach Service and Older Persons Enablement and Rehabilitation for Complex Health Conditions (OPEN ARCH). The aim of the service is to provide timely and appropriate care for the older person which will avoid unnecessary community deterioration in health, reduce avoidable hospital presentations, reduce unnecessary hospital admissions, and facilitate early and safe discharge from hospital, within the community setting.
About you:
In your new role you will......
* Contribute to an innovative evidence-based model of health care for aged care clients with complex health care needs.
* Demonstrate self-insight and awareness of the effect of own actions on others, to promote positive and supportive work environments, relationships and teamwork within the Older Persons Integrated Health Service, Physiotherapy and multidisciplinary teams.
* Deliver quality, patient focused assessment, intervention and case management, utilising high level contemporary knowledge and clinical experience.
About your opportunity:
In this position you will.....
* Provide clinical education to undergraduate students and multidisciplinary staff and contribute to staff development through clinical supervision, performance appraisal and facilitation of professional development activities.
* Deliver high level discipline specific clinical and consultancy services across all streams of the Older Persons Integrated Health Service.
* Provide quality Physiotherapy specific clinical advice regarding patients to professional and operational supervisors, relevant service managers and other stakeholders regarding professional standards and clinical service development.
