About the role
An exciting opportunity has emerged for an experienced and passionate Senior Social Worker to step into a pivotal role supporting the provision of a high-quality specialist rehabilitation service to clients of the Mid-Western Brain Injury Rehabilitation Program (MWBIRP) in Bathurst and the surrounding communities.
MWBIRP is a community-based program, providing case management services to adults with brain injuries, with a focus on supporting social re-connection to the community.
In this role you will be addressing a client’s reaction and adjustment to trauma, loss, hospitalisation and disability, implications for family and social relationships and welfare needs such as income, housing, legal, guardianship and access to community services. You will assist clients to develop healthy coping mechanisms, provide inclusion in the rehabilitation process and connect them to local resources and programs.
You will provide outreach services to the Bathurst Region, Lithgow, Oberon, Blayney, Orange, Cabonne, Cowra, Weddin, Parkes, Forbes, communities in Lachlan Shire and the Mid-Western Region.

As a case manager you will be an integral part of the multidisciplinary health care team, providing high quality services to patients / clients of the WNSWLHD in order to meet their health and wellbeing needs.
