Key Responsibilities
* Deliver home-based clinical OT assessments and interventions for My Aged Care (MAC) clients referred by ACAS and RAS
* Assess clients' functional ability, home environments, and suitability for home modifications or assistive equipment
* Develop, implement, and review person-centred, goal-directed care plans
* Ensure compliance with audit and accreditation processes, and contribute to continuous improvement efforts
* Provide specialist OT advice, maintaining adherence to regulations, policies, and quality standards
* Collaborate with the Allied Health and Community Access & Support teams to deliver effective client-centered services
* Communicate with internal and external stakeholders, ensuring high standards of service provision
* Support the Allied Health Administration Officer with intake tasks and daily operations
About You
* Bachelor of Occupational Therapy or equivalent qualification
* Minimum of three years' clinical experience as a Grade 2 Occupational Therapist
* Strong knowledge of My Aged Care CHSP and the Wellness & Reablement approach
* Experience engaging clients holistically to develop meaningful care plans
* Expertise in assessing and prescribing home modifications and assistive equipment
* Ability to work autonomously while collaborating effectively within a team environment
* Proficiency in client management software, technology, and standard office applications
* Strong time management, prioritisation, and organisational skills
* Exceptional interpersonal skills with the ability to liaise with diverse client groups and stakeholders
