Join Our Team as an Integrated Care Coordinator

We are seeking an Integrated Care Coordinator to provide care coordination, navigation and health coaching interventions within our multidisciplinary team. Using an interdisciplinary approach, you will deliver person-centred care that makes a genuine difference to patients living with chronic conditions in the community.

Remuneration: $74, $104,336.27 per annum, pro rata (+ super +17.5% leave loading where applicable)

Our ideal candidate will demonstrate:
* A tertiary qualification in Nursing with AHPRA registration as a Registered Nurse
* Assessment skills that identify the nursing needs and the application of those needs into a multidisciplinary comprehensive care plan that reflects the goals of the patient.
* Knowledge of patient safety, quality and risk and contribution to organisational quality and safety initiatives.
