* Supporting patients and families with advanced assessment, counselling and care planning that honours individual values and goals

* Providing professional leadership, supervision and advice that builds capability and confidence in others
Qualifications
* A recognised Social Work qualification with eligibility for professional membership
* Experience delivering complex, evidence‐based clinical social work services
* Strong communication and collaboration skills in multidisciplinary settings
* Confidence providing leadership, supervision and service improvement support
