Position Description: The position of the Support Worker is responsible for: Supporting patients to attend SEWB appointments Assisting to coordinate SEWB Team administrative functions as required Conducting client outreach as requested Supporting and facilitating group activities Supporting and facilitating the thorough care of Mental Health Referrals between TAIHS GPS, the QLD Health, SEWB team and, other relevant Mental Health Services Liaising with, and supporting clients to access the services applicable to wellbeing Preparing and maintaining reports / statistics associated with the role and responsibilities Qualifications & Experience: Certificate III Health Worker Community Services or Clinical Practice, or Qualification deemed relevant to the role Mental Health training experience (Desirable) After Hours Requirements: No Salary Information: Negotiable.
