About Our Case Worker Role
We are seeking a dedicated and compassionate professional to support children and young people in Out-of-Home Care through case management, therapeutic planning and frontline coaching.
Key Responsibilities
* Lead and review case plans to meet therapeutic and cultural goals.
* Coach and support staff to embed strategies in daily care.
* Engage with families carers and agencies to align goals.
* Maintain documentation and ensure compliance with standards.
* Promote safe nurturing and culturally respectful environments.
Requirements
* Diploma or working towards in community youth or social work.
* Proven case management and stakeholder engagement skills.
* Knowledge of trauma-informed and therapeutic care practices.
* Strong documentation and compliance capability.
* Current driver's licence and probity clearances.
