include:Working with or under the guidance of medical practitioners and Allied Health professionalsCollaborating with clinical and experienced colleagues and a very supportive admin teamEnsuring care plans are monitored and reviewed on a regular and ongoing basis and in response to changes in participant needsEnsuring services are delivered are in line with the person centred and consumer directed principlesMonitoring and evidencing or participant's wellbeing and documenting progress of participantsWorking withparticipants tohelpthem participate in the dailyactivitiesthey want to do, including hobbies, their interests, to moving about their home easilyEnsuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures, legislative requirements and program specific guidelinesAssist in participant service schedules including service planning, confirmation and short notice placements or cancellationsLiaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal, PRODA and other regulatory bodiesAssist participants to access other alternative services where the services are no longer meeting their needsUndertake administrative duties including data entry, processing invoices, following up participant enquiries, ensuring documenting all interactions ensuring compliance etc.Ensuring evidence is gathered and uploaded including quotes, confirmation of delivery of services etc.Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectivesWork with participants with their budgets and monitor participants' budgets in accordance with program guidelines.Communication with key stakeholders, internally and externally.Community Engagement:- engagement with key community stakeholders, promoting Just Better CareIdentify and participate in personal professional development.Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirementsAuditing of participant files regularly to ensure complianceConducting skills assessments of Community Support Professionals who perform complex care services as requiredDemonstrating a ongoing needs of the organisationMedication Administration and Complex Clinical Care e. palliative care, bowel care/suppositories, catheter care, stoma care, oxygen and moreUnderstand customer service needs or preferences, ensuring all relevant information is accessible for team members and service providers.Providing guidance to staff in relation to working with allied health professionals, monitoring and communicating the customer's condition, and other health and wellness related activities.Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.Manage communication with key stakeholders, internally and externally.After hours support to meet Just Better Care's duty of care to customers and support staff.Provide leadership in clinical risk management and clinical governance.Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures, legislative requirements and program specific guidelinesProviding training, coaching and guidance to Community Support Professionals, Enrolled Nurses and Support Staff/Case Managers as requiredConducting skills assessments of Community Support Professionals performing complex care services as requiredDemonstrating a commitment to best practice and evidence of continued professional developmentIdentify and participate in personal, professional developmentWhat you will need:Strong administrative, computer, time management and organisational skillsCurrent CPR / First Aid CertificateCurrent and satisfactory Criminal Record CheckCurrent and satisfactory Federal Police Check, NDIS Workers Screening Card& Blue CardCurrent Australian Driver's LicenceCurrent AHPRA CertificatePrevious experience in the community care or health/aged care sectorQualifications in Community Services, Human Services, Nursing or equivalent.Understanding of Community Nursing
