About this role:
* Deliver evidence-based therapy to customers living with disabilities, including assessment, intervention, reports and case management.
* Empower customers to achieve a lifestyle of their choice through a person-centred approach.
* Use the key worker model to collaborate with the multi-disciplinary therapy services team.
* Conduct hub-based and community visits.
* Liaise with families and external supports such as hospitals, agencies, schools and community services.
Key Responsibilities
This is a Full Time position (part time considered) based at our Coolbinia hub.
Essential Skills & Qualifications
* Tertiary qualification in Physiotherapy.
* AHPRA registration.
* Right to work in Australia.
* National Police Clearance within 6 months.
* Driver's Licence.
* NDIS Workers Orientation Module.
* NDIS Worker Screening check.
* Working with Children Check.
* First Aid Certificate.
* COVID-19 Vaccination Certificate.
